Accreditation fees are based on:

  • The number of patients you see
  • The number of sites you have
  • The time of year of your survey

For 1 center with less than 20,000 visits annually:

Between application and survey Annual Fee for 2009 $1890 minus $1700 paid above = $190 due
TBD Site Survey no fees at this time
Survey date + 30 days Post Survey Invoice $3375
January 1, 2010 Annual Fee for 2010 $1890
January 1, 2011 Annual Fee for 2011 $1890
Total over 3 years   $9045
