As a registered participant of the 2016 Fall Conference, you agree to all the terms and conditions set forth below including the use of photographic images. Noncompliance will result in registration cancellation without refund.

UCAOA is offering special rates on Exhibit Hall only passes to the 2016 Fall Conference: daily pass $50 member/$75 non-member or all-access pass $150 member/$225 non-member. Purchase your pass here.
The 2016 Fall Conference will be held in Nashville at the Gaylord Opryland Resort and Convention Center from Thursday, Sept. 29 at 3pm to Saturday, Oct. 1 at 12:30pm. Pre Cons will be held on Thursday, Sept. 29 and a Post Con will be held on Saturday, October 1.
Early Bird: members $800 / non-members $995
Regular Registration: Members $900 / non-members $1,095
Late Registration: members $1,000 / Non-members $1,195
After the Late Registration period, registration will only be accepted on site, pending space availability. Payment is required at the time of walk-in. We are unable to “hold space” for late registrations. Walk-in registrations will be at late registration pricing, plus an additional $100 walk-in registration fee.
Please note registration fees do not include any travel or hotel/housing costs.
Registered participants will receive an email prior to the start of the Fall Conference containing a passcode for accessing handouts and PowerPoint slides through the mobile app and laptops. Conference materials are available for download; UCAOA will NOT furnish printed materials.
Continue learning long after Fall Conference. Registered participants will receive complimentary recordings (PowerPoints synced with audio) six weeks after the event. Use these recordings to earn additional CME and/or UCMC credit or as a staff training aid.

If you are a member and see non-member rates at checkout, please call our office at 877-698-2262 before completing your purchase. If you purchase registration at the non-member rate and ask for a refund to reflect the member rate, you will be charged a $25 administration fee.
To qualify for a student discount, you must be a full-time student or resident. Proof of enrollment is required before registration can be processed. Email proof of status to UCAOA at email@example.com.
Spouse/Guests must pay for their own registration to attend educational sessions. If your spouse/guest would like to attend the evening Exhibit Hall Receptions, they can purchase a pass at the time of your registering or on site. The Exhibit Hall Reception pass includes hors d'oeuvre and beverage tickets.
Please note to uphold the professional and educational standards of the conference, children under 18 years of age are not permitted in the Exhibit Hall or educational sessions. Attendee Reception Guest passes are available for purchase for adults ages 18 and older. Only paid registrants are able to attend the courses.

If mailing a registration form, please note, the tuition rate is determined by the postmarked date. Payment must accompany registration form. NO checks will be accepted after September 1, 2016.
Cancellations received by August 22, 2016, will receive a full refund, less a $100 processing fee.
Cancellation received August 23 through September 16, 2016, will be refunded 50 percent of original price.
NO Refunds will be issued after September 16, 2016. There are no exceptions to the cancellation policy.
Please allow four to six week for refunds to be processed via original payment method. Cancelling your registration does not automatically cancel your hotel reservation(s); you must handle this separately.
UCAOA recognizes the importance of continuing education (and especially urgent care-specific courses) for our physicians. To provide CME to physicians, UCAOA must maintain our accreditation through the Accreditation Council for Continuing Medical Education. This process requires significant fees and staff hours to ensure that stringent guidelines are followed by UCAOA as well as all vendors and sponsors involved in our meetings in order to maintain our good accreditation standing.
Most other physician medical association conferences are funded or supported by corporations and/or pharmaceutical companies. UCAOA is working to align itself with corporate partners to offset attendee costs and corporate support has been growing but is not at a level yet to wholly underwrite the conference's expenses.
All physicians (MD/DO) must pay the CME fee. If you do not pay the fee at the time of registration, you will be charged an additional $50 administrative fee.
Photographic & Video Images Consent
Are children permitted in the exhibit hall and/or classrooms?
In consideration of your fellow attendees and the faculty, children (under the age of 18) will not be permitted in the classrooms or in the Exhibit Hall at any time. To arrange for daytime or evening child care, please refer to Child Care Services.
When do I get my Badge?
Badges will be distributed on site upon checking in at UCAOA Registration. Photo ID will be required to pick up your badge. Registration will be located in the Tennessee Foyer.
For security reasons, badge-swapping is not permitted. Anyone found wearing a badge that does not match his/her identification will be evicted from the conference without a refund. In addition, the badge/name will be cancelled. Individuals without a visible name badge will not be permitted into the educational sessions or Exhibit Hall.
Are the venues accessible to people with disabilities?
Yes. In accordance with the Americans with Disabilities Act, UCAOA uses only facilities that are in compliance. If you require special assistance please contact UCAOA by email at email@example.com or by phone at 877-698-2262.
What should I wear?
Attire for the meeting is business casual. Every effort will be made to provide a comfortable learning environment; however, meeting room temperatures can fluctuate greatly and may be difficult to control. The temperature is often cool in the meeting rooms, therefore, a jacket or sweater is recommended.
Should I bring my laptop/tablet?
Because of the way most course rooms are set up to accommodate many people comfortably, we do not provide outlets and tables in the meeting rooms. If you would like to bring your laptop/tablet remember to bring it fully charged each morning.
Is there a conference mobile app?
Course information and handouts are available via the conference mobile app. An email containing your access passcode will be will be sent out two weeks prior to the conference. To download the free app, search UCAOA Meetings in the Apple App Store and Google Play store.
When will I receive my CME Certificate?
You will receive your CME once you complete the overall Conference Evaluation through Survey Monkey (this survey is housed on the mobile app and will be emailed to all attendees at the close of the conference). There will be a link at the end of the evaluation that will take you to the online CME form. Once you complete the form, it will electronically submit to UCAOA. UCAOA will process it and the certificate will be available in your UCAOA online profile under Professional Development.
